EPF Form 11 is a self-declaration form that has to be submitted by an employee at the time of taking up new employment in an organization which offers EPF Scheme (Employees Provident Fund). It is used to auto transfer the money in the previous PF account to the new EPF account. Information pertaining to the employee’s earlier EPF account is required to be mentioned in the form. This declaration form allows the PF Department to maintain a comprehensive database with important details of the employees. It is especially useful during audits, inspections, cross-checking or verification of facts. In the past, an employee had to fill Form 13 to make the transfer to the new EPF account. However, after the introduction of Form 11, the automatic transfer request has been included in this form itself. An employee can make use of Form 11 to make declaration for both provident fund and pension scheme. All existing members are mandatorily required to fill up and submit Form 11, while it remains optional for new members.
An individual can download Form 11 through the website of The Employees' Provident Fund Organisation, India, Ministry of Labour & Employment, Government of India. Here are the steps to be followed to download the form:
Step 1: On the website, click on the blue-coloured tab called 'Miscellaneous'.
Step 2: Several options will appear. Click on ‘Downloads’.
Step 3: On the left side of the page, select the Return Form option.
Step 4: Scroll down until Form No. - 11 The Employees' Provident Funds Scheme, 1952 appears. Click on the Download button.

The member will have to provide his or her signature for the following undertaking:
1) Certified that the particulars are true to the best of my knowledge.
2) I authorize EPFO to use my Aadhaar for verification/authentication/eKYC purpose for service delivery.
3) Kindly transfer the funds and service details, if applicable, from the previous PF account as declared above to the present P.F. Account. (The transfer would be possible only if the identified KYC detail approved by previous employer has been verified by present employer using his Digital Signature certificate)
4) In case of changes in above details, the same will be intimated to employer at the earliest.”

After joining a new company, the employee is required to submit details through the new Composite Declaration Form 11. The individual will also need to provide UAN (Universal Account Number) and previous PF number. The employer will then enter the information as provided in Form 11 into the employer's portal. This data will be validated against the information available with the UAN. Should any discrepancy arise, the employer will have to verify or update the information provided. If the earlier UAN was Aadhaar seeded and verified, the transfer request made in Form-11 will result in auto-transfer of the accumulated funds in the previous provident fund account (PF ID) to the new PF account.
Once the transfer is complete, the new employee will receive communication via SMS on his or her mobile number seeded with the UAN and by Email (if registered). An important point to be noted is that the auto transfer of previous PF account is possible for Aadhaar verified employees only, according to news reports. If the earlier UAN has not been seeded with Aadhaar or the UAN was Aadhaar seeded but did not get verified, the member will have to apply for transfer in Form-13.
