EPF Name Correction Form

The EPF Name Correction Form is structured like a formal letter and is addressed to the Regional EPFO Commissioner. The below will give you an idea about its format.

Address it to the Regional P F Commissioner
Mention – ‘Joint declaration by the member and the employer’ or something similar as the subject line
Specify the name of the organisation where you were employed that had registered wrong information, and mention that your purpose of writing the letter is EPF name correction or any other changes that you may require like your gender or date of birth.
Thereafter, you will notice a table with three columns. Mention the information that you want to correct, enter the correct details and then the existing wrong information.
Apart from EPF name correction, this form is relevant for correcting other details like:
- Mistakes in your name, or that of your father or your husband
- Mistakes in your PF or EPS account number
- Incorrect date of birth
- Incorrect date of joining an organisation where you are or had been employed, and
- Errors in the date of leaving an organisation where you had been employed
You will have to fill the columns where you want the information to be corrected.
After you have filled up the columns accordingly, you will have to mention the documents that you will be submitting as proofs to support your request(s) for changes.
Next, you have to attest the form by affixing your name and then signing it.
Mention the name of the authorised signatory from your previous organisation.
Finally, the authorised signatory will have to sign the form and affix the company seal on it.

Why Do You Need To Change Incorrect EPF Details?

Inaccurate personal information in your EPF account acts as hindrances during withdrawal of insurance benefits, if and when required. For approval of a claim, the details given on the form should match the establishment records. Here are some reasons why you need to change inaccurate personal details in your EPF account.

Procedure to Change Name Online

Go to the official website of EPFO and login to your EPF account with you UAN number, mobile number registered with it and password. In case you do not have an active UAN account, you have to initiate an online request via the EPF UAN Helpdesk portal. UAN is a unique 12-digit number that is assigned to an employee and remains constant throughout his/her career.
Choose the ‘Modify Basic Details’ option available under the ‘Manage’ section.
Mention your Aadhaar number, along with other required details like name, date of birth, gender, as you have registered with Aadhaar.
After you submit the details, you can view the information registered by you, delete the request, check the status of its updates, etc. However, the request has to be deleted before the employer can approve it. You will not be able to change these details if these have already been verified by Aadhaar.

Name Correction due to Marriage

When an employee changes her surname after getting married, she has to fill up and submit a form to the EPFO Commissioner, along with a copy of the marriage certificate to support her claims. However, if you have not got your surname change incorporated in your bank account, you do not need to change your name in the EPF account.
