Form 10C is filled and submitted when you want to claim benefit under the Employee Pension Scheme (EPS). In simple terms, at the time of employment, the employee has a pension fund which he keeps secured with the government for his retirement. If the employee has changed his/her job, the employee can carry forward the same account to his new company. However, if the employee fails to find a job, the employee may apply for withdrawal of funds. Thus, in order to avail the benefits and at the same time, retain the membership with Employee Pension Fund (EPF) the employee files a Form 10C.
He/she can claim scheme certificate only when the overall service period has been more than 9.5 years and if he/she hasn’t reached the age of 50 as on the application date. Additionally, this scheme also allows you to carryforward the same when you rejoin in same or different organization. And therefore, a member can withdraw their PF accumulation through a scheme certificate.
The Withdrawal Benefit certificate is issued to an employee who has been in service for less than 9.5 years and hasn’t touched 50 years of age as of application date. This scheme intends for pension money back as the employee isn’t qualified for pension.
Note: This benefit is not applicable in case the membership is less than 180 days excluding the non-contributing period.
Form 10C is a 4 page document where the first two pages are required to fill in most of the cases and third page in case you have taken an advances against the account. There is no need to fill in the final page as it is used for administrative purpose.
The details on first page will consist of the following:
Name
Date of birth
Father’s name and/or husband’s name
Employer address
PF account number
Joining date with employer
Reason for leaving and date of leaving
Full address
The details on second page will consist of the following:
Date and signature, and some more details pertaining to age and account details for remittances
Particulars of family/nominees
Mode of remittance
The third page consist of details like the following:
Date and Signature
Details of wages and period of non-contributory service of member
Sum received
Make sure you write names in capital letter
Double check the date of birth
Avoid cutting or overwriting and in case of corrections, it must be attested.
The provident fund account number consist of region code (2 alphabets), office code (3 alphabets), code number (maximum 7 digit), extension (sub code) and the account number (less than or equal to 7 digits)
Serial no. 9 in the form should be filled if applying for scheme certificate, while Serial No. 11 can be left blank.
You should fill Serial no. 11 while applying for a withdrawal certificate, where Serial No. 9 can be left blank
Along with bank account details, a copy of cancelled/blank cheque to be attached, so as to ensure that the account number and IFSC code are clearly visible.
Also, in case of the member’s death, Serial no. 9 can be filled with details of the nominee/family/legal heir. In addition to this, Serial No. 10 and 11 should be filled by the respective legal heir/nominee/family member.
In case the member is drawing pension or family pension under the EPS scheme 1995, Serial no. 12 has to be filled.
The members of Employee Pension Scheme who satisfy the below criteria can apply:
In case you have left employment before completing 10 years and have attained the age of 58 years before completing 10 years of service.
An individual who has completed 10 years of service, but not attained 50 years of age or a member who is between 50-58 years of age and is not willing to settle with reduced pension.
Nominee/family of the member who has died before 10 years of service whose age was more than 58 years at the time of death.
Your will have to submit your application through your employer, under whom you were last employed. And if you have downloaded the application from epfindia website, all pages are to be attested in that case by both, the employer and the applicant.
However, if it’s a closed establishment, who’s Authorized Signatory/ Employer is not available any one of the following authorized official shall attest with his office seal.

In case you are applying for Scheme Certificate, Date of birth certificate, children of the member will be required.
Death certificate in case of death of the member
If the application is made by the legal heir Succession certificate is required.
INR 1 revenue stamp to be affixed into the form.

How to fill form 10C online?
The following steps will help you to fill form 10C:
Step 1: You can fill form 10C by signing in to the UAN Member Portal with your UAN and Password.
Step 2: Click on the ‘Online Services’ tab from the top menu bar and select Claim Form-31, 19 & 10C from the drop down menu.
Step 3: Post which the next page will show you Member Details, KYC Details and Service Details. Here, you can click on proceed online claim button.
Step 4: Here, you get redirected to the claims section and will find more details like the mobile number, UAN number, PAN etc. Also, you will have to choose the type of claim as withdraw pension only or withdraw PF only.
Step 5: Fill in the claim form carefully. Once you have finished completing the form, an OTP will be sent to your registered number. Once you enter the same in the form, the withdrawal will be initiated. Besides, when the claim form is submitted successfully, a notification via SMS will be sent on your registered mobile number. And once your claim is processed, the amount will be transferred into your bank account.
